• Care Coordination & Patient Support – Dynamic, person-centered assistance to help you stay on track with your health goals—whether it’s managing HIV, staying on PrEP, navigating STI or hepatitis treatment, accessing affirming care, or getting connected to supportive recovery and mental health services. Our team provides respectful follow-up and helps eligible patients connect to services that match their unique care needs.
• Behavioral Health & Recovery Services – Coordinated referrals to mental health and addiction treatment programs
