Navigation & Support
Our case management team works with you to understand your personal needs and connect you with supportive services. We help eligible patients explore insurance options and refer them to credentialed insurance brokers who can assist with enrollment. We also coordinate referrals to the Ryan White HIV/AIDS Program and a trusted network of community providers who offer services beyond what we provide in-house. Every referral is made with your goals in mind and your dignity at the center of the process.
